POSITION TITLE: Director of Nursing

DEPARTMENT: Inpatient Nursing; Subacute0, Primary Care & Outpatient Nursing

POSITION REPORTS TO: Chief Clinical Officer

GENERAL SUMMARY: In two to four sentences, state the major purpose, objective or function of this position.

The Director of Nursing (DON) sets the vision for Nursing Care at Polara Health and leads to ensure nursing services across the agency meet our values, standards and excellence in care. The DON works in collaboration with the Medical Director and other Clinical Directors to integrate care across outpatient, inpatient, subacute, crisis, intensive outpatient, community based and primary care. The DON tracks revenue and expense functions, data and trend reports, key performance indicators and uses the information from all three to strategically plan and implement best practices.

ESSENTIAL DUTIES/RESPONSIBILITIES: Briefly describe the work actually performed and list in order of importance. Begin each sentence with an action verb (e.g. plans, achieves, performs, administers, etc.). Focus on the purpose of the function and the goals to be accomplished by the position.

1. Provides clinical and administrative supervision of assigned Nursing Managers and supports other clinical and medical leaders and staff across the agency.

2. Provide fiscal oversight, data analysis, strategic planning and program development of integrated care services across the agency as pertinent to health and nursing services.

3. Hires, supervises, evaluates and discharges assigned Managers.

4. Participates in staff quality assurance, problem solving and planning committees as assigned as well as in community-facing meetings to represent Polara Health.

5. Performs other duties as assigned.


A. Supervision -

Directly supervises Outpatient Nursing Lead and Inpatient RN Manager as well as provides Subject Matter Expertise and cross-leadership support to other clinical, medical and non-clinical leaders.


A. Working Conditions (see pages 8 and 9): Office setting/Inpatient setting - some exposure to Level I Sub-Acute clients - Limited travel.

B. Equipment Operation (see pages 8 and 9): Computer / phone and pager.

B. Experience:

Field: Behavioral Health Leadership.

Number of Years: Minimum of 5 years clinical experience, 2 years supervisory

Special Skills/Knowledge: Ability to understand behavioral health financing, concepts of managed care; utilization management and quality assurance.
