months contract/ 13 weeks Shift : Monday to Friday, 8am-5pm &
Weekend rotation with call Job Description: Job Summary: We are
seeking a qualified Registered Nurse Case Manager with experience
in both inpatient and outpatient settings. The RN Case Manager will
be responsible for managing care coordination and case management
services for high-risk patients across the continuum of care. The
position requires collaboration with interdisciplinary teams to
ensure quality care, cost efficiency, and improved patient
outcomes. This role involves utilization review, care planning, and
coordination with internal departments and external providers.
Candidates should be skilled in identifying patient needs,
advocating for services, and promoting health and self-sufficiency
among diverse patient populations. Responsibilities: Conduct
comprehensive assessments and develop individualized care plans for
high-risk patients Coordinate services and referrals with
healthcare providers, behavioral health, substance use, and
community-based resources Monitor patient progress, modify care
plans as needed, and respond to emerging needs Perform utilization
review for inpatient and outpatient services Evaluate medical
necessity for referrals to specialty services not offered in-house
Collaborate with physicians, nurses, social workers, and care
coordinators to support continuity of care Promote efficient use of
healthcare resources and patient-centered care practices Document
all care coordination activities accurately in the electronic
health record Core Competencies: Associate Degree in Nursing
required Bachelor of Science in Nursing (BSN) must be obtained
within 2 years of hire Unrestricted RN license valid in any U.S.
state or territory Current Basic Life Support (BLS) certification
by the American Heart Association Minimum 3 years of clinical care
experience At least 1 year of travel nursing experience Prior
experience in case management, discharge planning, or utilization
review Proficiency in using electronic health records (EHR
systems)
