Practical Nurse (LPN) to fill openings in Mesa, AZ and surrounding
areas. Home Health LPN's must be licensed and able to reliably

the Home Health LPN · Effectively collaborate with care team ·
Patient assessment and evaluation of patients progress towards
goals · Implementation of care plan · Plan and facilitate
appropriate discharge · Accurate and complete patient documentation
· Provide care which promotes quality, continuity, and patient
safety · Provide skilled nursing care to patients in the community
· Compliance with Electronic Visit Verification (EVV) Requirements
of the Home Health LPN · Home Care experience preferred but not
required · LPN State License required · Basic Life Support (BLS) ·
Valid Drivers License · Current Finger Print Clearance card · 1
year or current TB test · Reliable mode of transportation ·
