Description of the Position:

The Lead Caregiver - Office will have the primary responsibility of filling in for shifts on selected days. The hours of availability will be between 7 a.m. and 7 p.m. on assigned days (see attached document for days). In addition to filling in for shifts, the Lead Caregiver will provide support to the office staff by completing general office duties.

Duties:

+ Understand and implement a client's non-medical needs in accordance to the established Plan of Care

+ Provides companionship for a client including, but not limited to:

+ Talking and attentive Listening

+ Providing social and emotional support

+ Promoting mental alertness through client engagement and activities of client's interest

+ Promotes a sense of safety and well-being (Notify the SYNERGY HomeCare office of safety concerns)

+ Assists in meeting nutritional needs by providing meal planning, preparation, and grocery shopping as needed by the client

+ Follow SYNERGY HomeCare's Infection Control Policies (notify the SYNERGY HomeCare office if in need of appropriate PPE or supplies)

+ Follow the telephony/mobile app/or other timekeeping methods instructions when clocking in and out for all scheduled shifts

+ Medication reminders (hand, explain, remind, read and open)

+ Observe and report any/all changes in the client's mental, physical, or emotional condition or home situation to the immediate supervisor in a timely manner

+ Establish and maintain effective communication and professional relationships with clients, family members, or other members of the client's support system

+ Participates in in-services and continuing educational programs, staff meetings, and client conferences as requested by immediate supervisor

+ Completes required documentation in a timely manner and submit to the SYNERGY HomeCare office according to policy

+ Utilizes any/all equipment and supplies safely and properly

+ Maintains confidentiality regarding client information (HIPAA)

+ Adhere to the SYNERGY HomeCare dress code policy as specified in the Employee Handbook

+ Report to work on time and ready to work for the day as scheduled shifts require

+ Other reasonable related duties as assigned

+ Adhere to all SYNERGY HomeCare policies and procedures as detailed in the Employee Handbook

+ Provide clients in assisting with activities of daily living by providing SYNERGY HomeCare approved personal care activities including, but not limited to:

+ Bathing

+ Dressing

+ Toileting

+ Incontinence care

+ Grooming

+ Oral hygiene

+ Schedule caregivers with clients and communicate the schedule to all parties

+ Document all caregiver and client communication in eRSP

+ Responds to questions regarding client and caregiver schedules

+ Confirm and update caregiver availability including vacation, sick time and holidays

+ Document skills and attributes of caregivers to ensure strong matching to clients

+ Update the office team on scheduling issues and HR recruitment needs

+ Perform other functions as deemed appropriate by the management team

+ Maintain computer schedules and ensure timely data entry for clients and caregivers

+ Follow-up with all clients on schedule changes

+ Confirm with Caregivers after accepting a Broadcasted shift

+ After-hours on-call

Required Skills:

Knowledge:

+ Knowledge of general office equipment

+ Knowledge of HIPAA laws

Education:

+ High school diploma

Experience:

+ 1-2 years of experience
