ones and other integrated service providers. GENERAL SUMMARY:
Functions as a member of the Vocational Services Team to assist
individuals with disabilities to enter or re-enter the work force.
ESSENTIAL DUTIES/RESPONSIBILITIES:
Provides job coaching and training to VR eligible clients in
community employment settings.
Contacts community employers for the purpose of job
development.
Provides consultations and support to employers.
Coordinates Vocational Services with CSP, the Community,
Rehabilitation Services, Administration (RSA) and other employment
related agencies.
Provides follow-up support to employed clients and their employers
per state regulations.
Works with and educates clients and their family members regarding
the impact of employment on their income and entitlements.
Maintains client records, reports and billing for (RSA)
services.
Provides classroom training to increase client work-readiness
activities.
Completes referral process for clients, who are identified as
appropriate for RSA services.
Completes monthly billing for services provided under RSA
contract.
Provides day-to-day supervision of other members of Vocational
Services Team.
Responsible for the production and distribution of lunches to
Polara Health Facilities under the direction of the Food Services
Supervisor
Ensuring WAT Kitchen trainees are abiding by Arizona Department of
Health Food Safety Laws and Procedures as well as providing
on-the-job training specific to clients' individual goals
Performs other duties as assigned.
JOB REQUIREMENTS:Minimum Education: HS Diploma or G.E.D.Minimum
Work Experience: Minimum 4 years of related education or experience
preferred.This position is considered Safety-Sensitive by Polara
health. EOE/Non-smoking facility. Pre-employment drug
