Job Description:
POSITION SUMMARYProvides care and services for adults with
multifaceted needs that may include stabilization in living
environment, coordination of placements, and support and services
when discharging from higher levels of care. Services provided are
primarily in the community and client's living environment.
Services may include comprehensive assessment and treatment
planning, solution focused therapeutic interventions, close
collaboration with clinical team to determine and guide service
provision. Position will lead a team to navigate and meet the
comprehensive needs of the client.ESSENTIAL RESPONSIBILITIESProvide
regular and consistent services and supports to clients identified
appropriate for community supports.Conduct comprehensive
assessments of adult clients to determine their mental health needs
and develop appropriate treatment plans.Provide short term
therapeutic interventions as appropriateManagement of clinical
treatment including crisis assessment and support as neededWork
closely with other members of the Community Care Team to insure
client's integrated and holistic care needs are metService
provision shall be conducted primarily in the client's living and
other environments as necessary or appropriateTrauma informed
service delivery approachProvide outreach and engagement when
neededClosely collaborate with other care teams within Polara to
coordinate and meet client needs (Inpatient Hospital, Crisis
Stabilization Unit, Outpatient Services, Primary Care)Attend Adult
Recovery Team Meetings (ARTs)Participates as a team member and
attends scheduled staffings and supervision.Partners with client
and team to identify treatment goalsMaintains clinical record per
agency policy and procedure.Attends all team meetings, case
reviews, staff meetings, supervisory sessions, and in-service
trainingsManagement of all aspects of treatment, including
referrals to additional resources when appropriate.Performs other
duties as assigned.QUALIFICATIONS21 years of age or older; or
minimum 18 years old and licensed or certified under A.R.S. Title
32 and providing services within the personnel member's scope of
practice; or minimum 18 years old for select administrative
positionsMust be able to provide or obtain an AZDPS Level One
Fingerprint Clearance CardMust possess and maintain a valid Driver
License, current automobile insurance, and a driving record that
meets the guidelines and requirements of the organizationMaster's
degree in related field (counseling, social work) licensed or
license eligible preferred2-4 years of experience working within
behavioral healthExperience working with individuals in crisis
situations and/or in need of intensive support services
preferredMust possess valid driver's licenseAbility to assess
situations and demonstrate critical thinking and problem solving
abilitiesAbility to delegate and lead clinical team as
