Job Highlights As a Hospice Social Worker, you will play a vital
role in our interdisciplinary team by: Participating in the
coordination of patient care. Contributing to comprehensive
assessments and care plans. Collaborating closely with physicians
and the interdisciplinary team to execute care plans. Ensuring
compliance with policies, procedures, state/federal regulations,
and accreditation standards. Qualifications & Skills Education:
Master of Social Work (MSW) from a program accredited by the
Council on Social Work Education Experience: Two years of social
work experience in a healthcare setting Other Requirements:
Excellent communication skills. Current CPR certification. Valid
driver's license and proof of automobile insurance. Equal
